HELPFUL TIP:

A deeply embedded fear often stems from trauma, so it can be helpful to explore emotional wounds that may be a factor. Your character might also become risk-averse, which can create problems in their relationships and limit their fulfillment. And for a character to achieve their story goal, they must overcome any fear-based thinking that's holding them back. The Character Builder is a great resource for pulling all these factors together.
NOTES:
Weaknesses make people feel vulnerable and ashamed. Whether the character is sensitive to a real or perceived physical deficit, lack of talent, mental deficiency, or other shortcoming, they'll arrange their lives around hiding it.

WHAT IT LOOKS LIKE:
Trying to appear physically large (standing tall, squaring the shoulders, etc.)
Exuding bravado
Refusing to accept fault or apologize for anything
Being quick to voice opinions 
Taking unnecessary risks
Keeping others at a distance to avoid scrutiny
Controlling others to appear strong or make up for a lack of control in their own life
Avoiding situations that will reveal the weakness (playing a sport, engaging in conversations regarding a topic the character is ignorant about, etc.)
Lingering in group settings so people can't talk about the character
Engaging in rigorous physical exercise
Criticizing or dismissing people who are strong in the character's area of weakness
Brushing off the concerns of others 
Refusing to see a doctor when an injury occurs
Suppressing symptoms of an illness or physical debilitation
Not accepting help from others; being staunchly self-sufficient
Being overly competitive with others
